To
apply in EAP.I form, duly filled in, with 4 recent
passport size photographs. Please ensure that
signature or thumb impression of the applicant
is put below the declaration along with two specimen
signatures OR thumb impression in the boxes at
the end of the form. In case the expiring passport
has been issued by another Passport Office or
Embassy /Consulate abroad, a personal particulars
form along with a photograph is also required
to be submitted along with the form. All application
forms and personal particulars form are available
at Counter No. 2 free of cost.
(Married applicants are required to endorse spouse’s
name in their passports; applicants started/stopped
wearing turbans, growing beards, ladies started/stopped
wearing hijab etc. are required to execute an
affidavit in this regard, for which an additional
BD 4/- would be charged per affidavit. In such
cases, the applicants are required to be present
at the counter in person to put their signatures
in front of the officers at the counter. Similarly,
persons changing signatures are also required
to be present at the counter.

of nationality (format available in EAP.I form),
copy of (i) lost/damaged passport, (ii) CPR Card
(iii) Loss Report Certificate (in original) from
Bahrain CID as well as Bahrain Immigration (only
in the case of lost passport cases) (iv) self-explanatory
request stating how the passport was lost/damaged. |
